PURPOSE/AIM OF THE STUDY: We report a case of a tumor composed solely of Leydig cell-derived from ovarian stroma! cells and not from preexisting hilus Leydig celi, cause of sexual disorder and hirsutism in menopause. CASE REPORT: Ovarian stroma! tumor solely of Leydig stroma! cells is rare, < 0.4% of ali ovarian tumors. lt can occur in young women, too, but almost always, it occurs in menopausa[ or postmenopausa! age. We report a case of ovarian tumor of Leydig cells in the medulla far from the hilus, derived from ovarian stroma[ cells rather than preexisting hilus Leydig cells, with complex and uncommon clinica! symptoms. In this report, we remark a) the importance of anamnesis, observation, and consideration of ali clinica! symptoms, b) the value of transvaginal ultrasonography for diagnosis, and c) the psychologist's usefulness's role in supporting the concerned patient with her existential distress and socia! discomfort. CONCLUSION: Ovarian cancer of pure Leydig stroma! cells only belong to the group of ovarian sterids secrete cancers, along with those of hilus Leydig cells, stromal luteoma, Leydig-Sertoli cells, and NOS tumors. They derive from different ovarian cells, ali of which are hormonal, generally benign, and rarely undifferentiated. Throughout her life, the patient has suffered an unrecognized hormonal dysfunction with psychological suffering for the facies and increasing weight in recent years. This work underlines the importance of a correct diagnosis to alleviate both physical and psychological suffering.
